If you would like to make a payment by credit card, Alverno Laboratories accepts Visa, MasterCard, American Express, and Discover.
Please remember when mailing to attach the payment stub, and include the patient’s account number on the check.

In order to efficiently assist you, please direct your call by selecting from the following options:

- If you are returning a call to a biller’s direct extension, select option 5, and then enter the extension number.

If you need to make billing information changes for a patient, please print, complete, sign and fax this form.
Be sure to sign the form before faxing it to the billing department.
Alverno Laboratories/Professional Clinical Laboratories Billing FAQs
Alverno Laboratories received demographic information from your ordering physician for your laboratory services. For further information about the specific nature of your billing, please contact Billing Services at 877-937-2190.
The laboratory report is sent to the ordering physician. Please contact the ordering physician’s office with any questions related to your test results.
Please allow your insurance company 4 to 6 weeks to process your claim. If your insurance claim has not processed in this timeframe, please contact them directly for assistance.
Your monthly statement from Professional Clinical Laboratories (Alverno Laboratories)Â will indicate if we have received an insurance response.
Your insurance carrier provides an Explanation of Benefits (EOB) to you that explains, in detail, your laboratory services. If you need further assistance determining the reason(s) why your insurance company did not pay a service, please contact them directly.
